To schedule a vaccine appointment by phone, call 844-876-2822 between 8 a.m. and 5 p.m., seven days a week.

To schedule a COVID-19 vaccine in Potter County, Pa., call 814-274-5460 (for adults) or 814-274-9198 (for pediatrics).

Scheduling A COVID-19 Vaccine at UPMC

By clicking "Schedule a COVID-19 Vaccine" above, you will enter UPMC's Certify Care system, where you will set up an account, answer important health questions, and pick a vaccination location, date, and time. You can search by location and which vaccines are offered. Once you schedule an appointment, you will receive confirmations and reminders from UPMC's Certify Care system. These may include text messages and emails from "UPMC Vaccine Scheduling."

Information on Third or Additional Doses of the COVID-19 Vaccines

If you received two doses of the Pfizer or Moderna vaccine, here is what you need to know:

  • If you are immunocompromised, including those who have had a solid organ transplant, are on chemotherapy, or have certain chronic illnesses:
    • The third doses can be received 28 days or more from the second dose.
    • The third dose should be of the Moderna (age 18 and over) or Pfizer (age 12 and over) vaccine.
  • If you are NOT immunocompromised:
    • You can receive the third dose if you are 18 and older.
    • The third dose can be received 6 months or more after the second dose.
    • The third dose can be any of the three available vaccines.

If you received one dose of the J&J vaccine, here is what you need to know:

  • Anyone 18 or older is eligible for an additional dose.
  • The second dose can be received two months or more after the first dose.
  • The second dose can be any of the three available vaccines.

Scheduling the COVID-19 Vaccine for Someone Under 18

  • The Pfizer COVID-19 vaccine is now available for children 5 and older. To schedule for someone 5 to 17 online, select the "Schedule a COVID-19 Vaccine" button above and request a "Pfizer pediatric" clinic.
    • If your child is an existing UPMC Children's Community Pediatrics patient, you can schedule an appointment online or by calling the office.
  • If you are under 18, or scheduling for someone who is, select a Pfizer clinic when booking online or request one when scheduling by phone. (A consent form signed by a parent or legal guardian is required for those under 18.)
